REGISTRATION FORMS
The following forms are required for registering your child. The due date for each form is noted.
- Tuition Agreement is due in spring for following school year
- New Family Registration Form (see our Admissions section for more information)
SCHOOL YEAR FORMS
The following forms MUST be turned in before the first day of each school year:
- Online Parent-Student Information/Emergency Contact Form (required on FACTS)
- Handbook & Policy Agreement Forms
- Medication Permission Form (one per student who needs medication.) The appropriate forms must accompany medications in their original, labeled container, before they can be administered.
- Asthma Inhaler Administration Form (required for students with an inhaler)
- Walking Field Trip (required for your child to participate)
- Online Student Dismissal Transportation Form (this required form indicates how your child will get home after school, selected during online registration on FACT)
- School Bus Transportation Form (optional). Needed if you require bus service.
- 4K Afternoon Agreement (optional for 4K-parents). Needed if you require afternoon care.
- Media Release (required for all families) Has your preference in this regard changed?

FAMILY FORMS
The following forms where filled out when you registered your child st St. Jerome school. They only need to be updated if the information for your family has changed. Please consider if you need to submit a new form at the start of each school year.
Immunization Record (required for students) - Has your child received updated immunizations?
Volunteer Driver Information Form (required for any volunteer who wants to drive on field trips)
- Has your insurance information changed?
- Have you purchased a different vehicle?
- Has your driver license number changed?

- Athletic Physical Card and Athletic Physical Alternate Year (contact students coach if uncertain which you need)
